I think this explains why my brakes were juddering so badly - a big crack right the way through the disc (towards the top of the pic between the two pads) and the pads seem to have disintegrated/broken up with chunks coming away from the backing plate:

Hybrid work may be on hold this weekend as I want to get these brakes fitted and the car back on the road again - might even be off back to the 'Ring again in October.
I'm hoping that this will solve my braking issues for good. 330mm AP two-piece discs, AP 6 pot calipers and Pagid RS29 pads...
