They are just like silencers that you bolt onto the end of your exhaust pipe if you think you are going to be over the noise limit at a track day/sprint/hillclimb.

Some exhausts have an adjustable one on the backbox section.

basically its a pipe which when bolted inside the tailpipe reduces your tailpipe size. Larger tailpipes tend to be louder, so it would make a 5 inch tail pipe into a 3 inch tailpipe and thus reducing the noise

Unless your exhaust is extremely loud I wouldnt bother getting one
